What to do when you go to the Doctor’s Office, Hospital, Urgent Care Center,
Lab, etc.?
-First find out if your provider is in your network (example GHI or Multi-Plan).

-If you are in an EPO or SPO plan you can only use in network providers.
-If provider is not in your network then you’re out of network benefits if applicable will apply.
-Present your Medical Id Card at ALL provider’s office/facility – verify they have the proper insurance info on file. ALL claims should be submitted to Employee Benefits Solutions LLC. ONLY!
-This will help you avoid collection notices from the provider and the labs those providers may use.
What to do when you go to the Pharmacy…
Show your Express Scripts Card.
When pharmacy keys in your script, the computer will tell the pharmacy staff your co-pay amount (based on brand or generic).
